A Novel Skin Grafting Technique with 1:100 Expansion: Xpansion Micrografting System

چکیده
Background: In case of large burn injuries, split-thickness skin graft (STSG) is limited by donor skin availability. It is not possible to achieve an expansion of more than 1:9 using conventional STSG. Here we introduce Xpansion micrografting technique, which allows a substantially larger expansion ratio enabling adequate coverage of large wounds. Methods: Pre-clinical and clinical studies using the Xpansion micrografting technique were reviewed in this study. In each pre-clinical study, the subject had a control wound and the wound healing outcome was defined by pre-determined parameters at specific time points. Clinical studies were reviewed to evaluate the efficacy of this technique in human subjects. Results: The Xpansion micrografting technology is able to achieve 100 fold expansion with wound healing quality comparable to STSG. In the pre-clinical studies utilizing this technique, the wound healing parameters such as the rate of re-epithelialization and wound contraction were significantly better than the control non-transplanted wounds. Multiple clinical reports demonstrated successful use of this technique in the treatment of large burn injuries with 1:100 expansion ratio and also in chronic ulcer wounds resistant to conventional therapies. Conclusions: This new technique has two main advantages: 1) In large wounds such as burn injuries, it provides an expansion ratio of 1:100, 2) In small wounds, it enables the wound care practitioner to graft the wound in an outpatient setting without the need for an operating room. The real expansion rate of meshers and micrografts: things we should keep in mind.
Skin graft expansion techniques (mesh and micrograft) are widely used, but there is ample evidence that skin graft meshers do not provide their claimed expansion rates. Although this finding might not be new for the majority of surgeons, less is known about surgeons' actual knowledge of expansion rates. The aim of this study was to evaluate the true expansion rates of commonly used expansion te...
متن کاملThe micrograft concept for wound healing: strategies and applications.
The standard of care for wound coverage is to use an autologous skin graft. However, large or chronic wounds become an exceptionally challenging problem especially when donor sites are limited. It is important that the clinician be aware of various treatment modalities for wound care and incorporate those methods appropriately in the proper clinical context. This report reviews an alternative t...

Introduction: Split-thickness skin graft (STSG) is the gold standard for treatment of major skin loss from major trauma or burn injuries. However, this technique is limited by the donor site availability in large burn injuries. Several techniques have been developed in the past to overcome this limitation but have been unable to achieve clinical relevance. With skin mincing technique, we have a...
متن کاملRepair of a Scalp Defect with Punctate Skull Fenestration and Skin Grafting
Reconstructing scalp defects can be challenging for a plastic surgeon. For this purpose, different procedures such as primary repair and coverage with skin grafts or flaps can be considered. Sometimes, the external table of the skull can be removed completely to the diploic space. At the same time or after the formation of granulation tissue, the diploic space can be covered with split-thicknes...
متن کاملBreast Reconstruction after a Bilateral Mastectomy Using the BRAVA Expansion System and Fat Grafting
SUMMARY Fat graft breast reconstruction following a mastectomy is always limited by the size of the skin envelope, which affects the amount of graft that can be injected in 1 session. Because the fat graft naturally resorbs in all patients, several sessions of fat grafting are necessary. BRAVA's negative pressure causes a "reverse" expansion of the skin envelope, thus permitting more space for ...
